The Home and Community-based Services (HCS) Program is a Texas Medicaid Waiver program that provides long-term services and supports to children and adults with intellectual and developmental disabilities (IDD).
Administered by the Texas Health and Human Services Commission (HHSC), the program helps individuals live in community-based settings rather than institutional facilities.
Eligibility is determined by the Local Intellectual and Developmental Disability Authority (LIDDA) and Medicaid approval.

Clinical Coordination
Ongoing collaboration with care coordinators, therapists, and healthcare professionals.
All clinical and day services are provided by licensed or approved contracted providers as authorized by the individual's plan of care.

A Host Home is a residential support model in which an individual lives in the private residence of a carefully screened and approved provider. Unlike a traditional group home, it offers a more personalized setting promoting independence and community involvement.
